Relevo de Responsabilidad

Yo, Padre, Madre o Tutor del Niño(a) que estoy inscribiendo y del que ostento la patria potestad, certifico que estoy enterado de las actividades de SPADI SOCCER ACADEMY a desarrollarse en el Colegio San Ignacio de Loyola en San Juan, PR o en la Academia Inmaculada Concepción en Mayagüez, PR y mediante la presente autorizo a mi hijo(a), menor de edad, a participar en la Academia de Fútbol de SPADI y en sus actividades recreativas y deportivas. De la misma manera reconozco que hay riesgos de lesiones en la práctica de fútbol por lo que acepto estos riesgos y acepto la responsabilidad personal por los daños que resulten de dichas lesiones. RELEVO DE TODA RESPONSABILIDAD A SPADI SOCCER ACADEMY, a Sports & Athlete Development Institue Inc. (SPADI), al Colegio San Ignacio, a la Academia Inmaculada Concepción, organizadores, auspiciadotes, directivos y empleados de cualquier reclamación por lesiones, enfermedad o incapacidad en la que se pueda ver afectado mi hijo(a) a consecuencia de las actividades en la Academia de Fútbol. Igualmente, me hago responsable de cualquier daño a la propiedad privada de SPADI Soccer Academy, del Colegio San Ignacio o de la Academia Inmaculada Concepción en la que se pueda ver involucrado el participante. Certifico que el participante ha sido examinado por un medico recientemente y esta físicamente apto para realizar actividades físicas y deportivas en la Academia. Autorizo a los directores de SPADI SOCCER ACADEMY a seleccionar un medico y/o hospital y que se le administre tratamiento a mi hijo en caso de una emergencia. Además, autorizo a la administración de SPADI, el Colegio San Ignacio, la Academia Inmaculada Concepción y/o auspiciadores a tomar fotos y videos de mi(s) hijo(s) durante su participación en SPADI Soccer Academy que serán publicadas en nuestra página de Internet (www.spadi.com) y/o documentos publicitarios de SPADI y sus auspiciadores u otros medios necesarios.

CERTIFICO que he leído el presente documento en su totalidad y entiendo su contenido, y que estoy de acuerdo con la totalidad del mismo. RECONOZCO que el presente documento es un relevo completo e incondicional de responsabilidad y que el mismo representa una renuncia de derechos y reclamaciones futuras, lo cual hago de manera libre y voluntaria, sin que fuese presionado u obligado a ello, en ejercicio de mi capacidad legal para así hacerlo.

Tras haber completado esta inscripción y aceptar los términos de este relevo de responsabilidad, me comprometo a entregar, antes de la fecha de inicio del programa deportivo, el certificado y/o historial médico de mi hijo(a), fotos 2x2 y cualquier otro documento que sea necesario para completar la inscripción de mi hijo(a).

Release & Waiver Form

I hereby represent and certify that I am the parent or legal guardian of the registered participant, who is under eighteen (18) years of age and who is in good health and has no special problems associated with his care. I certify that I’m aware of the activities of SPADI SOCCER ACADEMY at Colegio San Ignacio de Loyola in San Juan, PR or at Academia Inmaculada Concepción in Mayagüez, PR and I execute this release, waiver and indemnification for my child, myself and my heirs, successors, representatives and assigns. I hereby authorize my child, who is not of legal age, to participate in the SPADI SOCCER ACADEMY, including its recreational and sport activities. HEREBY understands and acknowledges that the ACTIVITIES OF THE EVENT(S) involve the risk of injury and/or serious injury and/or property damage. HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY HARM OR INJURY, OR PROPERTY DAMAGE arising out of or related to the Event(s), whether caused by the NEGLIGENCE OF RELEASEES, PARTICIPANTS, THIRD PARTIES, or otherwise, including, without limitation, the risks and dangers associated with practicing soccer or any sport activity. IN ACCORDANCE HEREBY RELEASES, WAIVES, DISCHARGES OF ALL LEGAL RESPONSIBILITIES AND AGREES NOT TO HOLD LIABLE OR SUE SPADI SOCCER ACADEMY, Sports & Athlete Development Institute Inc. (SPADI), Colegio San Ignacio de Loyola, Academia Inmaculada Concepción and each of their respective affiliates, and each of their respective directors, officers, employees, agents and representatives, or the promoters, sponsors, advertisers, participants, instructors, or any subdivisions thereof, officials, consultants, any persons in the Location or any Restricted Area of any claim by injuries, disease or incapacity sustained or suffered by my child as a result of the activities in the Soccer Academy. HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY PRIVATE PROPERTY DAMAGE to SPADI and/or Colegio San Ignacio or Academia Inmaculada Concepción arising out the participation of my child. I certify that the participant has been examined recently by a health professional and is physically apt and able to perform and participate in physical and sport activities in the Academy. I authorize the directors of SPADI SOCCER ACADEMY to select the means and/or hospital and to administer treatment to my child in case of an emergency. In addition, I authorize the administration of SPADI, Colegio San Ignacio de Loyola, Academia Inmaculada Concepción and his sponsors to take photos and videos of my child during his participation in SPADI SOCCER ACADEMY that will be published in the website (www.spadi.com) and/or advertising documents of SPADI and their sponsors or other necessary Media.

I CERTIFY that I have read this release and waiver of liability, fully understand its terms, understand that I have given up substantial rights by signing it, and have accepted it freely and voluntarily without any inducement, assurance or guarantee being made to me and intend my compromise to be a complete and unconditional release of all liability to the greatest extend allowed by law.

After completing the registration form, I make the commitment that I will submit the health certificate and/or medical file of my child, 2x2 photos and any other document that is required to complete the inscription of my child on a date prior to the beginning of the soccer program or summer camp.
